Registering with APOTHECARY BOTANICALS is a simple, 2-step process.

  1. Sign up online or complete the Application Form and submit by email patients@apothecarybotanicals.ca  or mail to: Apothecary Botanicals., 116-1776 Broadway St.,
    Port Coquitlam, BC
     

  2. Ask your Doctor or Nurse Practitioner to complete the Medical Document and submit it on your behalf, by email to patients@apothecarybotanicals.ca or mail: Apothecary Botanicals., 116-1776 Broadway St.,  Port Coquitlam, BC. 

    Your documents will be processed within 2-3 business days, and you will receive an email once your application has been approved.  For assistance with your application, please call 1-604-474-1795  and a Apothecary Customer Service Representative will be happy to help you.
